Your Guide to Starting Support with AESN Care

Starting the journey toward care and support can feel overwhelming — especially if you’re unsure where to begin. At AESN Care, we make the referral process clear, supportive, and easy to follow.

Whether you’re a family member, social worker, healthcare professional, or self-referring individual, here’s exactly how you can access our person-centred support services.

Who Can Make a Referral?

We accept referrals from:

Families and loved ones
Social workers or care managers
NHS discharge teams
Housing and safeguarding officers
GPs, OTs, or community nurses
Voluntary sector organisations
Self-referrals (private or direct payment)

🛠 Step-by-Step Referral Process

Step 1 – Make a Referral

Complete our simple online form on the Make a Referralpage.
You’ll be asked to provide:

Basic information about the person needing support
The type of service you’re requesting
Any current support needs or risks
Contact details for follow-up

Step 2 – Initial Consultation

One of our senior team members will contact you to:

Confirm receipt of the referral
Discuss needs, preferences, and eligibility
Answer any questions about services or funding

We’ll also explain how we collaborate with local authorities, CCGs, and other professionals involved.

Step 3 – Assessment and Planning

We’ll arrange an initial care needs assessment — either:

In person (at home or hospital)
Over the phone or via video
At one of our supported living settings

We’ll develop a draft care plan and discuss funding options (e.g. local authority, direct payments, private).

Step 4 – Matching the Right Team and Environment

Based on the person’s goals and preferences, we:

Suggest a suitable service and location
Match them with a care team trained in their specific needs
Agree on a proposed start date

This process is fully collaborative and person-centred.

Step 5 – Start of Support

Once everything is agreed and in place, we’ll begin:

Transition or move-in plans (for supported living)
Care at home or community-based visits (for domiciliary care)
Safety planning and welcome introductions
Goal setting and regular check-ins
